Michael, this is the cover to The Tormented by Dorothy Daniels (Paperback Library 63-034), published in 1969. I knew I'd eventually figure it out. Though I suppose as the owner of around 2 dozen Gothic covers myself, it better have been me, or I'd have been ashamed otherwise!

Dorothy Daniels was the wife of pulp writer Norman Daniels - while Daniels Career was in Decline in this period - hers took off and she wrote dozens of gothic paperbacks - I have read some of them and they ar quite good.
