Yep, I think all the strips that you and I have were commissioned by Derleth long after the original strip had ceased. I've never seen an original from the 1905-1927 time period. When Derleth found Condo in 1948, it was a year too late. In 1947 Condo had cleaned out his house and thrown away all his old stuff, which included some old strips.
It is a photo, actually. Ronald Clyne laid out and designed the cover, but I'm not sure if he took the photo or not.

The anthology does contain some mythos tales, including one of August Derleth's "postumous collaborations" with Lovecraft, "Innsmouth Clay".
