This is a 8.5 x 11 inch pencil prelim for "The Lighter Side of Doctors" written and drawn by Dave Berg. I believe this gag appeared in a MAD paperback. It features Roger Kaputnik, who was a self-portrait of Berg.

There was always a guy who had glasss, a goatee, and a pipe in his segments. So you did get the idea that he was putting himself as the straight man role with some deadpan reactions.

Berg liked drawing himself as Roger Kaputnik. Since he was poking fun at everyone else, I think he wanted to show he wasn't above being the brunt of a joke.
