The inside back cover of the program book from Phil Seuling’s 7th annual comic art convention (July 3-7, 1975) was set aside for autograph collecting.

This page is about as self-explanatory as it gets.

Kirby was a guest at the convention, and the book featured a Silver Surfer cover drawing and a Kirby interview.

I'd have liked this anyhow, but it's retcon commemorated to my 18-year-old self.

I got a concerned inquiry that this might have been a fake, which I appreciate. Checking in with 5 trusted sources, the initial consensus was 4:1 in favor of authentic. Then someone noticed the 1976 dedication on the big Hulk commission, and the chances that the 1975 piece was to the same fan at the convention tipped the balance the rest of the way… particularly the letter "B." I've dropped that Hulk piece into the additional images.
