Bane by his co-creator.

This commission shows Bane victorious just after he broke Batman's back (Batman #497) and then threw him down to the street from the top of a building in the first 3 pages of Detective Comics #664. Graham Nolan was the penciller for that issue and you can see this commission has the same setting as that brutal sequence.
