As if the swordfish battle weren't enough, he's also getting dragged down into the depths by a ball and chain! Maybe the fish can give him a ride back up.
He's got the knife to cut his ankles off and leave them for the sharp toothed buddy he is wrestling with(who I am sure informed him he will return them later!) while he surfaces before being eaten by other sharks in the vicinity, or dying through blood loss. Things were simpler in the forties.

Very strong theme with underwater peril and bondage! The full black background by Fine is used to highlight the curves of the swordfish that cuts through the composition and circles around Red Bee for even more visual depth.

True. Hit #5 was published when Frazetta was 12, so he probably saw and remembered it.
